The reason why Caestus, Spiked or otherwise, is so good right now is because it has a VERY quick R2 which does massive Poise damage. It staggers even Bull-Goat Armor users. You can usually do a follow up R2, which does the same Poise damage. It's not a true combo but it hits most of the time.
